Before and After-School Forms 2025-2026


These are all the forms necessary for enrollment in our Before and After School Program. To register your child for our program, please read through the Parent Handbook and DCF brochures thoroughly, follow the provided registration instructions (attachment below), and complete the registration form through our online system, My Procare.

HOW TO REGISTER

1. Download My Procare Registration Instructions, Parent Handbook (2024-2025), Discipline Procedures and Policies, Know Your Childcare Facility, and for VPK students, VPK Agreement.
2. Thoroughly look over the downloaded documents.
3. Follow the Registration Instructions provided on the "My Procare Registration Instructions" guide.
4. You will receive an email within 1-3 business days to confirm your child's enrollment in our program. Additional information will be provided after enrollment is officially approved.
